Engagement Coordinator.
Key Responsibilities
Coordinate, schedule, confirm, and manage virtual patient
appointments
Communicate with patients and their care teams through various
channels to ensure optimal clinical outcomes
Document all interactions with patients and families within
proprietary systems and EHR
Required Qualifications
Fluency in English and Spanish
2+ years of experience working in a healthcare setting
2+ years of experience coordinating care for fragile
populations
Experience communicating with patients via phone/telehealth
Knowledge of chronic care management processes
